Urinary Tract Infection and Aspiration Pneumonia Protocols
This page has primarily been created to provide our colleagues in local nursing and residential care homes with a single point of information.
Due to the high number of hospital admissions from residential and nursing homes for Urinary Tract Infections (UTIs) and Aspiration Pneumonia a task and finish group was convened to develop protocols for residential and nursing homes for these conditions.
The task and finish group comprised of GPs, Ambulance Service, Medicines Management, Clinical Nurse Specialists for Respiratory and Continence, Speech and Language Therapy (SALT) and residential and nursing home representatives. The new protocols, including relevant referral forms, are:

Posters have also been developed for use in residential and nursing homes as an aid for staff and to raise awareness about the protocols.
Falls Management Service
The Falls Management Service Pathway and Service Specification have been developed in collaboration with clinical staff from Staffordshire and Stoke on Trent Partnership Trust and clinical leads from the Clinical Commissioning Group.
The pathway has been redesigned to bring the service in line with NICE Guidance with regards to eligibility criteria for patients over the age of 65. Additional guidance is provided to support effective referral into the Falls Service, which is provided by Staffordshire and Stoke on Trent Partnership Trust.

Root Cause Analysis
Nursing homes that are commissioned to provide care are responsible for the safety for service users and must ensure that there are robust systems in place for reporting and responding to serious incidents, arranging and resourcing investigation.
Serious incidents that involve NHS funded residents commissioned by Stafford and Surrounds or Cannock Chase CCGs must be reported through the usual arrangements and also reported to the CCGs. In some instances the CCGs may require an Root Cause Analysis (RCA) to be undertaken.
The Toolkit below has been designed to support nursing homes when carrying out a Root Cause Analysis (RCA) investigation for serious incidents. It has been written in line with the pdf NHSE Serious Incident Framework (1.38 MB) , supporting learning to prevent reoccurrence (NHS England, 2015). There are also templates that nursing homes can use as a guide.
